Associate Membership Form - For individual membership.

Chapter Membership Form - A chapter membership is for an association. That association pays a chapter dues of $25 if It has its own 501©(3). If the association does not have its own 501©(3) and uses the CSCPA 501©(3), its yearly dues is $50. The association also pays $10 for each chapter member.
